Universal email 4 not sending form content??

I'm trying to use Universal Email to simply gather form content (i.e. contact us style form) and mail it to It's stated on your product information that this is possible:

Yes. Universal Email is simply applied to an existing form. Therefore, if you have created a contact form in Dreamweaver you can simply apply Universal Email to send the contents of the form via email.

However when I apply it I receive the email but with their is no body content??

I would greatly appreciate any direction to what I'm doing wrong.
