Validation not working

I have read all the posts in here and i'm stumped as to get the validation to work.
I have created an index.php page with a form, added capture, tried to validate all other fields.

The data is being posted to a database(mysql) Im using DWCS5. i have tried changing the post to index.php also to the thankyou.php page and neither show any kind of validation once Submit is pressed. Im have changed this from client side validation to SSV.

Any help?

Better tutorials on the workings of Form ToolKit would help.

you have a separate server validation server behavior applied for each form field, you should only have one server validation server behavior applied that validates all of the form field.

Select the validation type, select the server variable then click the plus button to add it to the list. Repeat for other form elements you wish to validate.
