Problem with new Installation CSS Form Builder

I just purchased CSS form builder this afternoon. I installed it with (seemingly) no problem. When I open Dreamweaver I get the following error (this is one of several errors):

"While executing onLoad in WAFB_AddForm.thm, the following JavaScript error(s) occurred:

In file "WAFB_AddForm":
initializeUI is not defined"

I have uninstalled and re-installed the extension twice. Even after I uninstall the extension and re-open Dreamweaver I am getting different error messages.

Can someone help me?

delete the Dreamweaver Cache file from the users configuration folder:
common_installation_issues.pdf

OK, did that. Now when I open Dreamweaver I get the following error:

The following JavaScript error(s) occurred:
Unable to openn script file: "C:\ProgramFiles\Adobe\Adobe Dreamweaver CS4\ Configuration\Shared\WebAssist\Library\WA_Configuration.js" (error3)

Then I click OK to that, and Dreamweaver opens, but not before showing this error 3 times (I click OK each time and then after the third time, Dreamweaver will open):

Unable to find WA_SiteTools_Progress.htm in the configuration/Floaters directory. The floater will not be shown.

Now what? I spend $500 yesterday for the "Super Suite" and the CSS Form Builder is the only thing I tried to install and I'm getting nothing but errors and problems - even after removing the extension. I don't think this Suite is very Super so far.

erorrs I am getting

I am getting same errors and I follow instruction BUT I can not solve problems. I delete dat file (WinFileCache.dat) but problem still coming.

Problems started after installing my UPGRADE extensions which are:
CSS Form Builder 2.0 which UNINSTALLED my other HTML extensions including DMXZONE HTML
HTML Editor 3.0.0
Form Toolkit 1.0

I need help

I think I am going to UNINSTALL all my webassist extension (I just count 60) then REINSTALL them which took 2 days last time (01/28/2009)

then promise myself one more time DO NOT BUY WEBASIST EXTENSION ANYMORE which I didn't keep my promise and bought some extension yesterday and create this problem

CSS Form Builder 2.0.0
HTML Editor 3.0.0
Form Toolkit 1.0.0

when installing the HTML Editor 3, you will need to use the Form builder 2 beta build that is available in the download center, if you do not use the beta build, you will run into issues like this.


Also after uninstalling extensions, you may need to reset your workspace layout. Go to Window -> Workspace --> Reset '<your layout name>'

I'm back! Having more issues. I am working on a new project and have just attempted to add a new form to this website. This is a php ecommerce site. I selected an area for the form, clicked "WebAssist" > Insert New Form. The CSS Form Builder: Add Form box opened no problem, I click the dropdown box under Choose a form and selected "Contact". This error message popped up:

Error generatiang form:
TypeError: this.themeArray[tGroup][tProp].toLowerCase is not a function

When I click "OK" to that, I get this error message:

While executing onChange in WAFB_AddForm.htm, the following JavaScript error(s) occurred:

At line 5119 of the file "C:\Program Files\Adobe\Adobe Dreamweaver CS4\Configuration\Shared\WebAssist\FormBuilder\shared.js"; dest has no properties

HELP!!! Anyone???
