Mememnto Error when starting DW5

I am getting the following error message...
Error in Mememnto Panel initializeUI:
Type Error: wa.user_interface.floaterShouldInitialize is not a function

I have cleared both Dreamweaver's site cache and removed the dat file but the errors still comes back.

When I go to the WebAssist's preferences panel the panel is completely blank so the there is no way to turn Mememnto Panel off.

what extensions do you have installed?

can you send a screen shot of the extension manager showing the installed extensions and versions.

Installed Extensions

Here is the list of Web Assist extensions that I have installed; please see attachment. As well, I also have Web Assist Dreamweaver 8 recipes installed.

there are a few issues I see:

1) Validation Toolkit, HTML Editor, Digital File Pro and Universal Email should be uninstalled completely, not just disabled in the extension manager.

2) The Dreamweaver 8 recipes extension is not compatible with DWCS5, neither is the eCommerce recipes extension.

3) Site Assist 3 is not compatible with DWCS5, Neither is Site Assist Pro 1 or the Constant contact join My Mailing list extension.
