checkbox and CampaignMonitor

I have necessity to create a form (a information request form) with a checkbox to subscribe to the campaginmonitor's newsletter... when a user click on the check box and send the request, its details are automatically sent also to the campaignmonitor's list.

Is possibile with Form Toolkit extension?

I Know that with API is possibile but I'm searching a tool that create it easier and faster:

No form toolkit will not create the code needed for this api, you would need to code that manually.
