Powerstore error on checkout

I am receiving the error message below upon checkout, is this a problem with the a Powerstore setup, UPS, or authorize.net?

"Transaction Not Completed
We're sorry
Your transaction could not be completed. Either your credit card information or billing address has problems and cannot be processed.
Please return to checkout and try again."

I'd add code somewhere on the page:

Based on the code dumped in this way you (or I) could determine why and where it is failing.

how do i update that incident?

Log into our web site and go to the my webassist section. You should find your support history there and you can click on the incident in order to reply.
