E-Mail Verification - User Name

I am looking for a way for the User Last Name to be included along with the email address that goes back to those in the WA Global configuration.

In other words, a means of identifying the email address with a name to go with it, so the administrator who is on the bcc will have a better means of identifying the registrant.

Is this possible?

and add code to display the other form fields you want added like:
