How do I insert form into existing html page?

How do I insert the form into existing html page? I already have a contact page designed and simply want to insert the form into a content div? The instructions are very scarce with this product I must say.
Thank you

copy all of the code from the contact form page that is above the doctype tag, and paste it to your existing page above the doctype tag.

copy the form from the contact page from the opening form tag to the closing form tag. pate this to your existing contact page.

Thanks Jason. I'm making a little progress. But form some reason the security code does not show when I view the page, so I can not submit the form. here is the page if you wouldn't mind taking a look.
I appreciate your help!

Thanks again for your help.
That folder was uploaded.
This is the message I'm getting:

Warning: main(/home/content/b/v/c/bvcwebdesign/html/WA_Universal_Email/BVCContact.php) [function.main]: failed to open stream: No such file or directory in /home/content/b/v/c/bvcwebdesign/html/WA_Universal_Email/WAUE_contact_1.php on line 43

Fatal error: main() [function.require]: Failed opening required '/home/content/b/v/c/bvcwebdesign/html/WA_Universal_Email/BVCContact.php' (include_path='.:/usr/local/lib/php') in /home/content/b/v/c/bvcwebdesign/html/WA_Universal_Email/WAUE_contact_1.php on line 43

The form again is at:
BVCContact.php

Thanks again and I look forward to your help!

looks like the error is coming out of the WA_Universal_Email/WAUE_contact_1.php file, can you send that one also.
