form not working

Hi I have designed my page and want to insert a form into my design. I have re-saved the page as a .php and put the form where I want, uploaded onto my site and seem to get this message: Parse error: syntax error, unexpected ')' in /vhost/vhost9/m/a/d/madantsport.co.uk/www/adventures_test.php on line 198

Can anybody help please. I have attached the code from the page.

kind regards

the page you sent does not have a form in it?

Please send a copy of the page that you are having the problem with.

do not copy the code into a word doc. instead, compress the original php page into a zip archive and attach the zip file to your message.

what version of form builder are you using to create this form?

What version of Dreamweaver?

Looking at the code, the issue is caused by the validation show if behaviors.

you can fix the problem by double clicking each on in the server behaviors panel panel and reselected the validation failure that should trigger it.
