Error messages everywhere!

I've just finished my first form with CSS Form Builder and so far, I'm NOT impressed.
Here are the error messages that flood my screen when I attempt to submit the form:

The first five error messages are causing the other ones.

The error messages mean that the upload folder you specified in the file up load portion does not exist, and the server does not grant high enough privileges for it to be created.

You will need to create the file upload folder manually and make sure that it has read and write privileges so that uploaded files can be written to it.

I have the same problem.

What kind of privilages do a folder need?

I am trying to create folder above webroot, I have tried with folder with 0777 and 1777.

I have apache dso the group and owner are accountuser:accountuser.

777 should be fine, if it is not working with those permissions assigned, you should contact the host for help.
