CSS Form Builder Update Form Javascript Errors

I keep getting the following Javascript Errors when trying to apply CSS Form Builder to my DataAssist Update.php page.
It occurs everytime on DataAssist Update pages, but DataAssist Insert pages always works ok.

I have also tried the following to correct the issue but does not work:
1. followed the instructions to delete the winfile cache
2. I have also tried to uninstalling/reinstalling the extension.
3. I have rebuilt my dw configuration folder, and reinstalled all extensions.
None of the above have fixed the issue. I still cannot finish building my update.php form. I have read all docs on modifying existing form with CSS Form Builder.

I'm attaching my Javascript error screenshots and my working files.

i have not been able to reproduce the error, but one thing to try would be to set the forms action:
action="photographer_category_Update.php?Id=<?php echo(rawurlencode($row_WADAphotographer_category['Id'])); ?>"

to:
action="photographer_category_Update.php"


then set it back after updating the form with CSS form builder. This is really the only difference between an update and insert form.
