Dynamic Forms

How do I go about creating a dynamic web form for an auto questionnaire? The desired results is to create a form with "as little" form fields as necessary but allow for the form to "dynamically add rows" as the user requires.
For example; Basic contact information is not what I am referring to..but instead the number of drivers, the number of vehicles, the number of moving violations, the number of accidents.

As I see it, if I created a "static" form for "up to" four drivers.... prospects with more than four drivers would not be able to complete the submission.
If I create the form for 8 drivers...the form would be massively long!

The form would ask questions such as; How many divers? Response say...6...then the number of tables for collecting driver info. expands from "say 2" to 6."

How do I go about creating such a form? I do not know how to use create a record set, I do not know how to create data bases, I have hosting with php scripts and mysql and I do not know php?

Css form builder does not create this sort of form. You would need to create the form manually using jQuery. We do not offer support for creating this type of form.
