Help with form 2 upgrade

I purchased the form 2 upgrade this morning and since installing it I have been receiving java script errors. I am running windows 7, CSS 4, SUITE Extensions from 04-2010.

I tried to open the this link on firefox and chrome but the page display blank.

I have uninstalled HTML EDITOR, UNIVERSAL EMAIL 4, and I am not sure if Validation tool-kit came with my suite. Also I have uninstalled form 2 and I am getting the java script errors.

right click the link and select save as to save it to your desktop.

it is a pdf document, so you will need adobe acrobat reader to open it.

What is the exact text of the javascript error you are getting. We need to know the error to have any clue on the cause.
