How to add image and button form elements

How can I add an image (type=image) and/or button (type=button or type=reset) form element to a form. It's missing in the list of available trypes when I add a new field. I'm using DW CS5.

the form will only be created with one button, the submit button.

By default, it will use type="submit"

to convert the submit button to an image, click on the edit css button, from the main select list, select buttons, then choose an image for the button.

you can add a reset button to the form after it is created by going to Insert -> Form -> Button, then changing the action to Reset in the property inspector.
