Captcha not accepting correct code

Greetings!

I am currently working on a form that is an appointment request.

appointment.php

When I go directly to the bottom of the page (when it loads) where the captcha is, it comes up immediately with the error message "Entered text does not match; please try again".

When I fill out the form and try to submit it, it comes back as invalid=true.

I have tried deleting the entire form and recreating it using form builder, but it is still not working.

Any ideas or help would be appreciated!

I just tried a test of your form and it submitted just fine, I am not able to reproduce the issue.

Hi Jason,

WOW that was fast! LOL! I was just in the process of posting that I had found my own error and was in the process of fixing it.

The error turned out to be with the Captcha IMG. I set it up on another form with custom size and height as well as changed the background color (saving myself from having to go back into CSS Form). As such it seems I forgot to change the ID name to match the Captcha field.
