Trouble receiving form with Universal Email 4

I am having trouble receiving the generated form from the quote page with UM4. It was working fine with 3. I have attached the Webassit directory, quote page and email form I should receive. Could you please set me on the right path. I (bbgd@sbcglobal.net) and the quote requester and I am the person to receive the quote. This is just for testing purposes. What else do you need?

I fixed it. I had the wrong SMTP address. Sorry to take you time on this silly issue.
