File Manipulation Upload not working

I have a data assist Insert and Update form. When I apply the new UE4 File Manipulation Upload Files behavior, it will not send the file to the server. I have made sure of directory permissions, and I don't receive any error messages. Documentation on the WA Site is mixed and I can't find anything specific to using this new Files Upload function with Data Assist. Is there updated documentation on this, and/or can someone describe to me how to make this work?

to upload a file, your forms encoding type must be set to "multipart/form-data". change the form tag:

<form action="reEnrollment_Insert.php" method="post" name="WADAInsertForm" id="WADAInsertForm">



to:

<form action="reEnrollment_Insert.php" method="post" enctype="multipart/form-data" name="WADAInsertForm" id="WADAInsertForm">
