how do i set up more than 1 contact form per website?

I have a website with a contact us page, and another page to request a free pass (but its the same as the contact us page, but needs different email info). How do I use this solution pack for 2 pages in one website?

you need to make a copy of the contactus.php page for the second contact form. and also make a copy of the WA_Globals.php file, save that as WA_Globals2.php.

In the second contact form, change the include reference:
<?php require_once('WA_Globals/WA_Globals.php');?>

to point to the second globals file:
<?php require_once('WA_Globals/WA_Globals2.php');?>

the session being created by the captcha script is created as

if you change the session being used in the validation, you will need to modify the cde that sets the captcha image:

<img  src="WA_ValidationToolkit/WAVT_CaptchaSecurityImages.php?noisecolor=<?php echo $WAGLOBAL_Captcha_Noise?>&bgcolor=<?php echo $WAGLOBAL_Captcha_BG?>&textcolor=<?php echo $WAGLOBAL_Captcha_Text?>&transparent=<?php echo $WAGLOBAL_Captcha_BG_transparent?>&characters=<?php echo $WAGLOBAL_Captcha_Characters?>&width=<?php echo $WAGLOBAL_Captcha_Width?>&height=<?php echo $WAGLOBAL_Captcha_Height?>&font=<?php echo $WAGLOBAL_Captcha_Font?>" alt="security code" />

and add a filed querystring variable:

for $_SESSION["captcha_2"]:

<img  src="WA_ValidationToolkit/WAVT_CaptchaSecurityImages.php?field=2&noisecolor=<?php echo $WAGLOBAL_Captcha_Noise?>&bgcolor=<?php echo $WAGLOBAL_Captcha_BG?>&textcolor=<?php echo $WAGLOBAL_Captcha_Text?>&transparent=<?php echo $WAGLOBAL_Captcha_BG_transparent?>&characters=<?php echo $WAGLOBAL_Captcha_Characters?>&width=<?php echo $WAGLOBAL_Captcha_Width?>&height=<?php echo $WAGLOBAL_Captcha_Height?>&font=<?php echo $WAGLOBAL_Captcha_Font?>" alt="security code" />
