UE400 Java Script Error

Hi -- UE400 is not working fine when I tried to make some edits. I have since cleared the WinFileCache several times. It still did not help. Please review the attached graphic to see the error message.

Make sure that all of the form elements on your page have a type attribute to them.

I have seen this error occur when there are form elements that do not have a type attribute:

<input name="fName" id=fName" value="" />

This should be changed to:
<input type="text" name="fName" id=fName" value="" />
