Content area not showing and JS error

Content area not showing and JS error

The content area isn't showing at all below the buttons on the page. When I tried to go back in to edit the form, I get a javascript error (which by the way is not an uncommon experience while using my WA extensions). I did delete my wincachefile.dat file or whatever it's called and re-started my DW CS5 to no effect, still get the error. Any thoughts on fixing the situation are appreciated. See the attached screen capture of my error message.
