Problem with CSS Menu Writer

Hello friends, how are you! I have a problem with CSS Menu Writer, as you can see in the attachment that I send. The point is that I have reviewed the connection to the database and do not see disadvantages of any kind. Here, in addition to the attachment will transcribe the code to connect to the database, which is right about the login data for the same. Could you please help me solve the problem soon.

It looks like you may not have uploaded the CSSMenuWriter directory to the same relative path that it was on when you created the files on your local drive. Can you post a link to the page that you have the menu applied to so I can take a look at it?
