Yes, you should be able to add two different forms onto the same page without any issue.

Yes, you should be fine adding the map as well. Make sure your cursor isn't in the form when you're adding the map. Are you getting an error message when you insert the map?

Not getting the error message, but couldn't completed.
On step 5, click on ' include directions' and try to get the code from Dynamic Data but doesn't show ' simple form' only show---simple contact, server variables and WA validate entries......I had this map on few pages but now forgot everything.....getting old

thanks again for your help Jimmy

Can you attach the page that your form is applied to so I can take a look at it? Maybe the form code is being added in a strange way so it isn't being recognized as a form.

This just means that the API key that you are using for the map is different from the one that is needed for your domain. If you are testing on localhost, you can use the one we provide you in the help documents. Otherwise, you wll have to generate your own:
signup.html

thanks Jimmy,
Sorry to asked you two questions, my first question was .....

On step 5, click on ' include directions' and try to get the code from Dynamic Data but doesn't show ' simple form' only show---simple contact, server variables and WA validate entries.....

thanks n i attached the file

I wasn't sure if you had gotten past that question, since you applied a map to the page. Looking at your page, there is only a SimpleContact_NewFromBlank_Default form on the page, so that is why there is no other form bindings when you click on the dynamic data.
