JavaScript error

First I create a simple form in the page "contsct us". Them I put Universal Email and its work well. Them I want to record this information in a mysql table and apply "insert record" server behavior, but this message appear:

While executing applyServerBehavior in InsertRecord.htm, a JavaScript error occurred

I already delete dreamweaver cache (.dat) but the error continues.

Now it worked. I copied the important parts of the page for the Note Pad and deleting. After I created a new page and copied back the files. But before you copy the part referring to the Universal Mail, I applied the Insert Record and then copied the code from the Universal Mail.
