Won't send "submitted file" attachments

This replaces the final item on my earlier thread because I haven't had a response and am now getting desperate.
Hi again,

Emails are being delivered fine and so are "entered value" attachments but submitted files using the file field are not.

The site is and I attach grabs of the screens I took as I set it up and a zip of the whole site.

Maybe some parameter is missing from my file field?

to be able to upload a file in a form, you must change it's encoding type.

change the form tag:
<form method="post">

<form method="post" enctype="multipart/form-data">
