Survey Builder Documentation

I can't seem to find any documentation regarding the use of the Survey Builder scripts. I have the Dreamweaver 8 Recipes book and the CD that came with it. I've been through the book from cover to cover but didn't find anything.

I've installed the recipes extension and see the SurveyBuilder snippets in the snippets panel.

Is there any additional documentation on how to use that set of scripts or is it pretty much self-explanatory?

The survey builder recipe was included in the Dreamweaver MX2004 Recipes book. it was not included in the Dreamweaver recipes 8 book, however the files for the recipe where added to the CD mistakenly.

Survey Builder Documentation

Hi Jason, This may seem a bit odd. Coming back to this thread after 11 odd years. I recently dug out the Web App Recipes(Still relevant) and wanted to re-visit the app structure. All fine except the Survey Builder. I have all the templates and snippets. However, due to the time lapse, mysql date is not working as well as it should. Please do you know how I can get hold of the completedFiles (Survey Builder ONLY).
Anyone else who reads this and can help. Will be appreciated.

Unfortunately Survey Builder was discontinued and doesn't work on the latest versions of Dreamweaver. What are you looking for from that extension? Maybe I can help you achieve the results you are looking for without that tool.

This actually refers to Survey Builder Recipe - included in Dreamweaver Recipes Book - Joseph Lowery/Eric Ott.
Seems the issue is more problematic. I'm trying to build the Survey Builder Recipe from scratch in Dreamweaver CS3, with the snippets installed. I followed the step by step instructions from the 2004 Recipes Book by Joseph Lowery/Eric Ott.
Too many issues with mysql/mysqli clashes. I was hoping someone may have the completedFiles of just the Survey Builder Recipe. Could help me compare what I did wrong with my attempt and possibly make corrections.
The whole exercise helping me with php learning curve.
