Fatal Errors

I have the form uploaded, but I am receiving these errors:

Fatal error: main() [function.require]: Failed opening required '/var/vhosts/i/insafe.ca/public/www/ContactForm_GMC/WA_Universal_Email/Templates/contactus.php' (include_path='.:/usr/local/lib/php') in /var/vhosts/i/insafe.ca/public/www/ContactForm_GMC/WA_Universal_Email/WAUE_contact_1.php on line 43

Line 43 in the WAUE_contact_1.php is:

require_once(dirname(__FILE__) . ("/Templates/contactus.php"));

I would appreciate your help. I thought I might save some time and energy by buying a ready made contact form. Silly me!

sounds like the template file was not uploaded.


make sure the entire ContactForm_GMC/WA_Universal_Email/ folder is uploaded to the server.

If you continue having problems, please post a link where i can see the issue to investigate the cause.

All the files are there on the server. I double checked the paths and the files seem to be in the correct location. The link is:

In the Contact From files, there is a Template folder inside of the WA_Universal_Email folder, this folder has not been uploaded to the server. Go to:
WA_Universal_Email/


there should be another folder in there named Templates which is missing.

Thank you Jason! I guess I thought those files were being pulled from another Template folder that I have uploaded. Everything is good now!
