Confirmation Checkout Page Not Working

I am receiving page not found error at the final checkout stage. I can not debug as I do not understand it. Any help would be appreciated. This is what is in the URL.

%3Cbr%20/%3E%3Cb%3ENotice%3C/b%3E:%20%20Undefined%20index:%20%20../PHP_SELF%20in%20%3Cb%3E/home/content/admin/confirm.php%3C/b%3E%20on%20line%20%3Cb%3E855%3C/b%3E%3Cbr%20/%3E

It is saying something about an Undefined index: on the confirm.php page on line ???

I used the checkout wizard to make the pages (FYI). It made the pages in the main directory and I moved them to the admin/ directory even though the initial page was in the admin/ directory so this might be an issue. Don't know.

Solved the issue. It was on this line of code on the form.

<form name="ecart_checkout_form" action="<?php echo $_SERVER["../PHP_SELF"]; ?>" method="post">

As you can see it is the $_SERVER["../PHP_SELF"] which should have been $_SERVER["PHP_SELF"] this of course happend because I moved the page into another directory.
