Can't add upload to DA Insert page

I am using CS4 and the latest DFP plugin on a Windows 7 (32bit) intel PC. I created a form using DataAssit and need to have 1 upload field. I read the article "http://assets.webassist.com/how-tos/dfp_da_insert_update_ht.pdf" and have completed the steps down to step 11. I have the Insert page open but when I try to select "Upload" from the WebAssist > Digital File Pro menu, I can't because the option is greyed out. The only option on the DFP menu that's not greyed out is "Progress Bar".

I've tried recreating the form, but am still having the same problem. You assistance in solving this problem woul dbe greatly appreciated.

If the option is grayed out then it means you have an html file open or not site defined. Make sure to have a server side file open before applying DFP.
