applyServerBehavior error in Multiple Record Inserts

I keep getting this error when trying to apply Multiple Record Insert.

While executing applyServerBehavior in WADA_MultiInsert.htm, a JavaScript error occured.

I'm following the online video tutorial exactly...

Any suggestions?

try deleting the Dreamweaver cache file from the users configuration folder:
common_installation_issues.pdf

I closed Dreamweaver, deleted the webcache file, then restarted Dreamweaver. I'm still getting the same error message. But I did notice it was adding it to my server behaviors. (attached pic). I can open that behavior up and do edits, but when I "finish" it give me the same error. The form won't submit.

Update:

I started over without a Template, hand made the form instead of CSS form builder.

I don't get the error when I use Multiple Insert, but still haven't figured out why it's not inserting into db.

I was able to get it to work. It seemed weird. In the "configure bindings" area, I had to assign a repeating field to make it work. Before I was putting all static data in, except for the primary key to test.
