Choice of Recipients

I'm am building a simple contact form which I have done a few time with good success. This time however I want to add either a check box or radio buttons or something to choose the recipient for the form. It doesn't make sense to have to create a separate contact page for each person so I assume this has been taken into consideration. But I cannot find anything in the documentation about this. Giving it a shot to see if I could do it intuitively points out my own personal limitations. Obviously I'm not as bright as the average WA user because for the life of me I cannot figure out how to do this. I guess it's a sign I'm getting older. Could you point me in the proper direction to find the instructions on having a choice of recipients for the form to go to?

To do something like this, you can use a select list with the name and the email address as the value. You can either get the names and populate the select list from a database, or manually insert the values into the select list. When the form is submitted, in your email object, you would want to bind the address to the value of the select list. If you are using Universal Email to send the email, in the wizard, you would click on the lightning bolt next to the to and choose the form field of the select list.
