Emails not being sent on registration etc.

Hello - I have just published my USR solutions pack to a 1&1 server that I am using to "learn"
I have used DW CS3 to publish the site.
All the files and folders in the solution pack are in the root of the site, I have attached a jpeg of the remote site files and folders.
I have a number of problems.
1 - First one is to do with Registering. Using page users_Registration_Simple.php I get the following error
"Fatal error: main() [function.require]: Failed opening required '/homepages/16/d91521753/htdocs/dgdme/WA_Universal_Email/Email_Templates/registration.php' (include_path='.:/usr/lib/php') in /homepages/16/d91521753/htdocs/dgdme/WA_Universal_Email/WAUE_users_Registration_1.php on line 43"

I looked at line 43 but could not see what was missing though I do see that this page refers to WA Globals which sets up a number of parameters including the site domain name in the Root_URL, the From email address and the mail server. Those three data are correct and I have even tried leaving the Email server info as "" and using the auth.smtp server info provided by the 1and1 host. I have not provided a screen grab of WA Globals as it now contains sensitive info such as passwords etc.

2 - This brings me to issue 2 that follows on from issue 1. Interestingly the registration takes place and the user is intered into the MySQL database. (I checked using the Admin feature) When loggng in as the new user page users_profile.php opens. Clicking Verify produces the message that a verification email has been sent but no email is actually received so is most likely not sent - I see this as part of the overal email send problem.

3 - The user profile page itself has some interesting (read challenging) aspects.
3.1 - Phone number is not accepting UK Mobile format nor a single string of numbers. eg a phone number nnnnnnnnnnn
3.2 State: is not accepting N/A
3.3 Buttons not appearing indicating a broken image link

4 - page users_Registration.php the CAPTCHA code is not displaying and again this looks like a broken link.

problem 1 is beyond my understanding - what did I do wrong?
problem 2 I figure is driven by problem 1?
problem 3.1 sound like it is a format problem - how do I fix that?
problem 3.2 is beyond my understanding - what did I do wrong?

To me problems 3.3 and 4 seem to be basic web links but as I have not altered the page code I figure these should work "out of the box". But they are not.

Would appreciate being taken by the hand here and pointed in the right direction.

The error message = line 43 code

Line 43 refers to "/Email_Templates/registration.php (see attached jpeg screen grab) but the site does not have a folder Email_Templates nor a page registration.php

I am going back to the backup folder of the solution to see if I am missing the Email_Templates folder.

Progress Update Emails not being sent

Slightly red face now because I discovered that my site was in fact missing the Email_Templates folder - so I used WinZip to extract the folder and uploaded it to the live server.
Result! The server now sends an email to a new user and the registration process completes fine so the following is a status update on my challenges.
Problem 1 Fixed! by uploading the missing folder
Problem 2 Half-fixed .. using the "log in" for a previously registered user (when the email system did not work) and when I click send verify, I still get the email sent message but no email is received. I thought that this was a spam catcher on the email address that the verify message was sent to but not so. I will try and delete the user and re-register.
Problem 3 - 3.1 and 3.2 now appear to work OK - did nothing! do not understand!
Problem 3.3 and 4 are still present - some button image links not working and the captcha graphic does not display.

So any thoughts on broken image links in forms and pages that I have not modified will be much appreciated?

Problems fixed

OK the last update and I continue with the saga because there are valuable lessons here for others who may make similar mistakes. However, as I have been working with html for a number of years and recognise basic issues, I figured it had to be that I was missing files.

Being new to PHP I also struggle to display pages in glorious detail on localhost so I did not pick up locally the missing images until I published the site to the live server.

Anyway what I did was go back to the WebAssist solution pack original files, that were secure on a USB drive as the original Zip file, and using WinZip (recently installed) I compared each folder and each file one by one. Yes! there were a significant number of files missing from the web site. These were not determined on the testing server localhost because I did not have a localhost mail application - in addition I put captcha not displaying down to the localhost not connecting to the internet (firewall).
Anyway having located all the missing files and folders, and installed them on my local site, I then uploaded them to the live server.
It could be that the whole issue was caused by using the inbuilt decompression application in Windows Vista. When I originally unpacked the solutions pack it seems that various folders went astray!
I cannot think of a single reason why this should have happened but it has re-inforced my belief in two things.
1 - WinZip beats the Vista zip tool hands down!
2 - Murphy's Law (if it can go wrong it will!) is alive and well (with apologies to all folk called Murphy!)

Npw I can read through the rest of the Forum to see how to change those pesky US States for UK Counties and how to organise a refresh button for Captcha without refreshing the whole web page.
