Parsing Errors

I am getting parsing errors when I try to use your CSS Form Builder. I am using Dreamweaver CS4. I added the simple contact, and simple appointment defaults, and it appears as if the form enters incomplete code.
The first error message I get is:
Parse error: syntax error, unexpected T_STRING in reservations.php on line 10

I look at the code and see its missing semicolans at the end of each validation statement on the following fields:
WAValidateDT((isset($_POST["Contact_me_group_Pickup_Day"])?$_POST["Contact_me_group_Pickup_Day"]:"") . "",true,"","","",false,"","","",true,1)
WAValidateRQ((isset($_POST["Contact_me_group_Name"])?$_POST["Contact_me_group_Name"]:"") . "",true,2)
WAValidateEM((isset($_POST["Contact_me_group_Email"])?$_POST["Contact_me_group_Email"]:"") . "",true,3)
WAValidatePN((isset($_POST["Contact_me_group_Mobile_Phone"])?$_POST["Contact_me_group_Mobile_Phone"]:"") . "",false,true,true,4)

So I add the semicolans and test again, and get the following error:
Parse error: syntax error, unexpected ')' in reservations.php on line 137

Basically this piece of code is incomplete as well:

if (ValidatedField('reservations','reservations')) {
if () {
if (!(false)) {
?>

I am not really sure what to put in that code. please advise...

I have tried creating dozens of forms from the simples custom fields, to simply using the simplest ready made contact fields or appointment fields you offer, and no matter what, I get the same errors.

Its been a HUGE waste of time, money and learning curve trying to get the CSS Form builder to work for me in CS4.

I am hoping you can help point me in the right direction.

Because its taking me so much time to try to get past this issue, unless you can help, I will need to go back to doing forms the simple way, without the CSS Form Builder.

Could you please send me your page in a zip file. I just need the page with the form on it. This will help me debug the issue.

I just did a quick test with a simple contact form and a single appointment form with no errors, did you do any modifications to the form or did you just use the quick insert to create those forms?
