Creating a fare quote form

Dear All,
I am trying to create a fare quote form. I have created a db with the various fields. The form contains three form fields( City, Zipcode and airport) and a search button. When my client enters their zip code or city and airport and after clicking on search button, the client will be able to get the Price(fare) from say Airport to City/Zip code or vice versa.
Any help how I can use Data Assist to achieve this?
Any help will be appreciated.

DataAssist have a DataAssist Search Wizard that will help in the set up process.

First you will need to have a record set to pull all information from your table. And then launch the search wizard. for the search page you would select the page that includes the form. then you will click the Add button to add your search options.

In your case you will need to add three different search criteria, one for each field in your form.
