Cannot Upload File - or Create Folder with KFM

Cannot Upload File - or Create Folder with KFM

I have inserted HTML Editor in the admin area of a site. It loads fine, and I can browse a specific folder structure (which already existed) with KFM, and even load images from those folders.

Problem is, when I try to upload a file it looks like it is starting but then it gives me an error "error uploading file - no file uploaded".
Permissions should be fine - I use this with an older editor without any problems. The files I am testing are not very big, anything from 1k to 100k and I have tried plain text files, jpg, png, gif, etc.... Nothing will upload.

Also - something I cannot seem to find is how to create a sub folder within this structure I have. I can't even find the option to do it?
(EDIT: I have found the method for creating sub folders, and that is working fine. I just can't seem to upload any files)
