Security Assist SQL problem

SQL problem

So I tried to follow your solution and am still receiving errors. It keeps referring to a nwrainbow.php that I have never had created and do not currently have either.

I have included some information - this is the error message and I have attached a screen of my myPHPadmin.

MySQL Connection.
Connection Name: Corries
MySQL Server: 127.0.0.1
User name: root
Password: I assigned in system
Database: passwords (this is what they are called in the MyPHPadmin - not sure what is right but I have tried to use passwords, corrie passwords, and corrie in this field to no avail)

This is the error message I receive when trying to access this page after I have posted on my remote server.

I really need to figure out what is the hangup in my programming in order to go live with the website this weekend. Any help you can provide would be great. Thanks!
