On the checkout page, add the following select list:

then on the confirm page, locate the collection of hidden form elements near the bottom and add another one with the same name as the select list to capture the value posted from the checkout page:

then in the UPS Server behavior on the confirm page, click the lightning bolt next to service type and select the UPS_Service form element.
