My forms won't work in Wordpress

Hi,

I'm trying to set up a contact us page in my wordpress site: contact-us but it resolutely refuses to work. I've added the form in its entirety to the contact template that I've set up, but it just produces php errors every time. The first are just locational errors for the webassist/email folder, but when I sort that one out the errors just keep coming thick and fast.

Does anyone have experience of using Universal email on WordPress? I'm using version 3.1.1 of WP and 4.0.0 of UE. Help!
