Select each of the form elements. In the Property inspector, click the lightning bolt next to initial value and select the corresponding item from the validated entries collection in the dynamic data bindings.

In the case of checkboxes, you need to click the dynamic button in the Property inspector. click the lightning bolt next to Check If and select the corresponding item from the validated entries collection, then enter the checked value of the check box as the Equal To value.
