the checkout page is trying to load the page:
WA_ValidationToolkit/WAVT_ValidatedForm_PHP.php

but it is not on the server.

Upload the WA_ValidationToolkit/WAVT_ValidatedForm_PHP.php file to the server so the checkout page can load it.
