Errors Using Form Builder in a Site Sculptor Site

Problem: Receiving error “Error initializing Add Form UI: TypeError: wa.generate.checkHTMLEditorV3Plus is not a function.” (See screen pic).

Situation: Platform XP SP3, DWCS4. Site created with Site Sculptor, have CSS FB Build 23 installed along with HTML Editor 3.0. Running mySQL and Apache on XAmpp testing server locally.

Site created with SS, including database creation/connection login. On a non-content page, attempting to add a form . Clicking on the WebAssist- Forms-Insert New Form throws the above referenced error. Have also experienced the same problem with a blank/non-template page.

Have also deleted cache file with no change in error.

Upon clicking “OK” at the error message, the CSS FB window opens, but there are no forms or designs to choose from. If you attempt to create your own, you get multiple error messages, one of which I’ve attached.

Please help guide me as to what I am doing wrong.
