validation toolkit showing fil structure in form field on ecart checkout page

Dear webassist,

I am using form toolkit to validate the checkout.php from ecart, the validation is working as I placed the validation on the confirm.php. But what I am getting is when I want to fill the form field with the submitted information or the information from the recordset, it is not showing up and is showing a file structure instead of the what is usually created on the forms that I have created before the same way.

This is what I am getting:
