Possible Third Party Conflicts Causing Error

I have created a booking page that has a booking form that was built using CSS Form Builder. The booking form is created on a separate page and included into the booking page using Framework Builder.

In the booking form right-hand column I have some thumbnail photographs inserted using a third party lightbox Dreamweaver extension. Everything works fine on all the remaining pages of the website, except for the booking page. When clicking on the thumbnail, the enlarged picture opens in the parent window but without the lightbox feature and effect.

When I remove the inserted booking form originally placed using framework builder, the enlarged photograph opens with the lightbox feature.

The same principle is on the contact page where this works with no error.

I suspect that somewhere there is some conflicts in the code, and being a novice, I can't figure out the error. Thanks in advance for your help and pointing in the right direction.
