form not working

form not working

Hi

I have recently bought form builder and installed it successful on my mac. I have gone through the tutorials to build my form and have produced a quick test form and uploaded it onto my site.

This is the error message that is comes up with once I submit my form. I appear to being having difficulties in the form sending to my email address can anyone help please. I no pro!! so please simple explanations would be really appreciated.
