CssFormBuilder2 Download Center

I have created a download center and believe everything is working excecpt that nothing happens when I click the download button on the download_center.php page for the user. I can see all of the details etc. but nothing downloads. I have checked the permission on the folder containing the file to be downloaded (it is an htm file) and have verified the path. I am using Dreamweaver CS3. Would appreciate your guidance
