Garbled code out of the zip file

Unfortunately, I'm now having a problem with adding my template to the file. It doesn't seem to matter whether I use DW's "Apply Template to Page" or just copy/paste. I always get this error:

  Fatal error: Call to undefined function validatedfield() in /nfs/c05/h01/mnt/69578/domains/yumemedia.net/html/qprod/contact.php on line 83  

When I copy/paste, it sites line 63. When I "Apply Template to Page" it's line 83.

Is there something else special to do to add a template to this form? I've been using this form a couple of years now but, this last update has been giving me the blues. LOL

Edit: Unfortunately, at this moment, I just got a text stating that MediaTemple is having an issue with my part of the server so you may not be able to view my pages right now. Just keep checking back. Thanks :-)
