Upload / File Management - Locations are wrong!

I just ran a quick test using the same steps you provided, and the Advanced Images Upload worked correctly. The images are always being stored in the upload folder inside the 'test' folder.

The only thing that could be different here is where you have your DW set up. Do you have your site set up in the root or is your site set up in the root of the sub folder.
