JavaScript Error UE4 & CS3, MySQL

Hi

I am constantly getting an Javascript Error when I try to set up my UE4 to populate my visitor table in my MySQL database.

It works when I use the UE4 option to create it's own - but this doesn't give me what I want as I want to capture the individual fields into the database. When I try to map them using UE4 I get the Javascript error when I press "Finish" after updating it all. Also it doesn't save my mappings either.

I have cleared the cache as per instructions, after each time it has happened, but this has not resolved this problem.

Would it be better to not use UE4 to do this or can you see what I am doing wrong?
