Order goes through, but

We are able to successfully place an order (getting the proper confirmation from Authorize.Net, etc) but rather than the ‘checkout-success.php’ page appearing afterward, it goes back to the ‘confirm.php’ page. Can anyone see the attached code (confirm.php) and pinpoint the error?
