Did you fill in all of the address values in the WA Globals file:
$WAGLOBAL_Address = "<street address>";
$WAGLOBAL_City = "<city>";
$WAGLOBAL_State = "<State>";
$WAGLOBAL_Zip = "<Zip>";
$WAGLOBAL_Country = "<Country>";
none of this info is being passed in the request, except for the Country:
