404 code when loading in drmWv CS5

Hey webmasters and coders, I have created a member_database using MySQL and phpAdmin off of my web-domain server. When I try to upload it in DreamWeaver CS5 using the database connection, then select MySQL Connection. I followed all the steps and I always get the "http error code 404 file not found".
I called up my service provider and they check my database and server connection and said everything is good on that side. I even went to the extreme and called adobe and had them check the dreamweaver side and said they were fine.

I purchase SecurityAssist and want to use it but the above is kicking my butt. Can anyone in the community help!!!! please, thank you
